DES MOINES, Iowa (AP) - A Ku Klux Klan application form has been taped to the locker of a black student at a middle school in the Des Moines suburb of Urbandale.

The Des Moines Register reports that 14-year-old Dante Johnson-Taylor found the form May 9 on his locker at Urbandale Middle School. He told his mother, Tara Patterson, and she's spoken to principal Daniel Meyer.

Patterson says her son was hurt by the incident and says "It's not a joke." Patterson says Dante told her the student who taped the form is someone he thought was a friend.

School officials wouldn't talk about whether the student who taped the form was disciplined. Patterson says Meyer called to help arrange an apology from the student.
